Practice Policies & Procedures
Payment is expected at the time of service. We accept most form of payments including major credit cards and pay pal. Package plans are available. Most new patients will receive new patient email with required forms prior to their initial visit. These forms must be completed in advance of your appointment, or we require you to come to your initial30 minutes early to fill the forms. Failure to complete the required paperwork may require us to reschedule your appointment.

Appointment Cancellation
In order to be respectful of the medical needs of other patients, please be courteous and call our office promptly if you are unable to make your appointment. It is important to cancel your scheduled appointment, we require that you call at least 24 hours in advance. Appointments are in high demand and waiting lists, and your early cancellation will give another patient the possibility to have access to timely medical care. Preparing for Your First Visit
Fill out the forms and be as descriptive and detailed as possible. Email it back to us and bring a copy to your appointment as well.
